Important Information:
The Learning Assistance Lab (LAL) coordinates all special test accommodations for persons with disabilities in credit and non-credit classes at Monroe County Community College. The LAL also arranges special test accommodations on the placement tests used by the College, the ASSET and the COMPASS.
THIS FORM IS TO BE USED TO REQUEST A
CLASSROOM TESTING ACCOMMODATION:
To arrange for a test accommodation in a class at MCCC, the student must
first be registered in the LAL (with acceptable documentation on file)
as a student with a disability FOR THE CURRENT SEMESTER, HAVE
SEEN AN LAL COUNSELOR IN THE CURRENT SEMESTER, and HAVE TAKEN AT
LEAST ONE TEST WITH AN ACCOMMODATION IN THIS CLASS ALREADY. No test
accommodations will be arranged using this online request form until you
have met these requirements.
In order to use this Test Accommodation Form, you must have first received a username and password from the MCCC Disability Services Counselor.
THIS FORM CANNOT BE USED TO REQUEST
AN ACCOMMODATION ON A PLACEMENT TEST!
To arrange an accommodation on a placement test, a prospective student
must make an appointment with a counselor in the LAL (in person at Room
C-218, Campbell Bldg., or phone (734) 384-4167). The counselor will explain
what is needed in order for a person to become eligible for accommodations
on placement testing.
Instructors at MCCC do not provide test accommodations for students with disabilities independently of the LAL! Please do not ask your instructor to do so.
WHEN REQUESTING A TEST ACCOMMODATION USING THIS FROM, PLEASE KEEP IN MIND THAT THIS IS ONLY A REQUEST. YOUR COMPLETION OF THIS REQUEST FORM IN NO WAY GUARANTEES THAT A TEST ACCOMMODATION WILL BE SET UP. THIS REQUEST WILL ONLY SET IN MOTION THE TEST ACCOMMODATION PROCESS.
If you do not receive a response from an
LAL staff member within 1 business day, it is possible there has been
an error in the receipt of your request. If that happens, please contact
the LAL (384-4167) immediately!
IF YOU ARE REQUESTING AN ACCOMMODATION
THAT IS DIFFERENT THAN WHAT YOU HAD BEFORE IN THIS CLASS, DO NOT USE THIS
FORM!
CALL THE LAL (384-4167) FOR AN APPOINTMENT WITH A COUNSELOR AS SOON AS
POSSIBLE.
